Kitsap Mental Health Services (KMHS) is actively recruiting for a passionate and dedicated Developmental Disabilities Care Coordinator to join our team.
As a member of our multidisciplinary team, you will play a crucial role in planning, developing, coordinating, and providing treatment, rehabilitation, and support services to our program clients.

Primary Responsibilities:
Serve as a liaison between Developmental Disabilities Administration (DDA) and other community resources.
Facilitate communication between KMHS care coordinators and DDA staff for treatment continuity.
Provide direct services, including individual, group, and family counseling, crisis intervention, and case management.
Demonstrate knowledge of the current Diagnostic and Statistical Manual of Mental Disorders.
Coordinate treatment with internal and external substance use treatment providers.
Assist clients in gaining resources and education to support their autonomy and basic needs.
Work collaboratively with community partners and advocacy support groups.
Provide crisis intervention counseling and coordinate with Designated Crisis Responders when appropriate.
Document client progress according to established policies and procedures.
Consult with families and community agencies to coordinate the treatment process.
Establish and maintain effective relationships with KMHS and DDA staff.
Minimum Qualifications:
EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.
EXPERIENCE: Minimum of 1 year of job-related experience.
The capacity to work well with children and families.
Preferred Qualifications: In-depth knowledge of and experience with I/DD population, outstanding communication abilities, collaboration and administrative skills, and a high degree of professional insight and knowledge.
