JOB SUMMARY:
Administers skilled nursing care under the supervision of a Registered Nurse (RN) and the provider directed plan of treatment.
Provides nursing services in accordance with applicable state licensure, in compliance with agency policies and procedures, applicable health care standards, and governmental laws and regulations.
Supplements the nursing care provided by the RN.
Responsible for inspecting a patient's home environment for possible hazards and dangers, and educating the patient and any caregivers about the patient's condition and needs.
May be required to work weekends, evenings and on-call.

Per Diem Premium 15% (on base plus steps, in lieu of benefits)

QUALIFICATIONS:
Education
Graduation from an accredited school of practical nursing.
Experience
Three years LPN experience.
Community/home health or medical/surgical experience is preferred.
Licensure/Credentials
Washington State Licensed Practical Nursing license.
Basic Life Support (BLS) certification within 30 days of hire.
Valid Washington driver’s license.
Proof of insurance on automobile used for work.
Driving record in compliance with OMC standards currently in effect or as may be amended.

Reliable automobile transportation to travel to patient residences.
Must have a cell phone (portion of cost reimbursed by OMHH).
Must have computer internet connectivity (OMHH work laptop provided).

Paid Sick Leave – PSL is accrued at a rate of 1 hour for every 40 hours worked and is available to use after 90 days of employment.

Full-time Employee Holidays – 6 fixed holidays and 4 Floating holidays.
Floating holidays are allotted annually on your plan entry anniversary date.
Part-time Employee Holidays – Part time holiday bank (equivalent to 10 days per year for a full time employee); use by request or receive payout of 8 hours upon reaching 24 hours in bank.
Paid Time Off – Beginning accrual equivalent to 12 days per year for a full time employee.
Pro-rated for part-time employees.
